Proctored Exam Appointment Request

Please fill out the appointment request below for your proctored exam; there is a $30 fee due on the day of your exam. Be sure to follow-up with your college/institution to ensure you've completed the necessary requirements for them as well.

The Testing Center will email you a confirmation to indicate your appointment has been scheduled. If adjustments are needed, or if the staff have any follow-up questions, they will contact you. Please be sure to provide your best contact information on the form.
