ADA Exam Appointment: Testing Center

Please fill out the appointment request below. Appointment requests should be submitted 3-4 days prior to your exam date, or sooner, whenever possible.

The Testing Center will email you a confirmation to indicate your appointment has been scheduled. If adjustments are needed, or if the staff have any follow-up questions, they will contact you. Please be sure to provide your best contact information on the form.
